2010/2011 SCHOLARSHIP APPLICATION
DEADLINE FOR SUBMISSION: FRIDAY, JUNE 18, 2010
Dear Student:Florida Thoroughbred Charities, Inc. (FTC) is a 501(c)(3) Corporation and the charitable arm of the Florida Thoroughbred Breeders’ and Owners’ Association (FTBOA). This letter contains important information that you need to know before applying for an FTC scholarship.
Scholarships are available for students each year and are based on the following criteria:
▪ Applicant MUST be a Child, Employee or a Child of an Employee of an FTBOA Member
▪ High School Grade Point Average (3.0 and Above) or Last Semester of College (Must Maintain a 2.8 GPA During Scholarship Period)
▪ Official Transcript Must Accompany Application for Consideration
▪ Financial Need (Total Household Income)
▪ Full or Part-Time Student
If you would like to be considered for a scholarship for the 2010/2011 school year, please complete the attached application, sign and date and return to: FTC Scholarships, Attention: FTC Scholarships, 801 S.W. 60th Avenue, Ocala, Florida 34474, on or before Friday, June 18, 2010. Important: Please answer all questions and submit an official high school transcript, as well as transcripts from any other colleges or trade schools which you have attended. The FTBOA Membership Affiliation Information must be furnished, as well as the information concerning your Financial Need/Total Household Income. Incomplete or illegible applications will not be considered.
Please do not submit more than one application per academic year. In addition to any transcripts, you may attach up to one additional sheet of paper if required. It is not necessary to attach resumes or letters of recommendation. A properly completed application will expedite our selection process. You will be notified in writing if you are selected to receive a scholarship.
